Our most recent follow-up questionnaire COVID-19 Survey is now closed.

Thank you to the thousands of participants that completed this survey online.  To those that attempted the online survey but were not able to do so, we thank you as well for your efforts and for your feedback on how we can improve our processes going forward.

Your ongoing involvement in Alberta’s largest health research study is crucial to ensure that the huge database continues to grow and support cancer and chronic disease research.
