Welcome! This tutorial page is dedicated to helping participants register and get started with our latest online follow up questionnaire COVID-19 Survey
This registration process involves two Steps:
1. Sign up for a basic account at MyAlberta Digital ID (MADI). This account acts as a secure online doorway for you to enter the ATP participant portal
2. Connect your basic MADI account with the ATP participant portal. This action will enable you to complete the COVID-19 survey online.
To help you with this process, we have:

Step 1 Checklist items
Go to to the MyAlberta Digital ID (MADI) “Create Account” web page by clicking on this link: https://account.alberta.ca/signup
Establish your sign in information by setting a Username and Password, and entering a unique Email address.
Please remember your specific username and password as you will need these very soon.
A suggestion to simplify this is to make your Username the same as your Email address.
Continue to fill out the remaining required fields for Personal Information and Address. All fields are required unless they say “(optional)” beside them.
If you successfully submit your web page, you will be prompted to go back to your email inbox and find an email from MADI.
Return to the email inbox used with your MADI account. Find and open an email sent from MyAlberta Digital ID (email@example.com) with the subject “Confirm your email address”. If you cannot see this email, please refresh your inbox or check your junk mail folder.
Open this email and click the top link to confirm your email address and finish creating your basic account.
You will be taken to the MADI “Confirm Account Creation” web page. Enter your newly created MADI Password, and click “Next”.
You will be taken to the MADI “Manage My Account” web page. This is the end of Step 1.
Step 2 Checklist Items
Return to the email inbox used with your MADI account. Find and reopen the invitation email we sent you from Alberta’s Tomorrow Project (firstname.lastname@example.org) with the subject “New COVID-19 survey from Alberta’s Tomorrow Project!”
Go to the instructions for Step 2
Click your personal link found in the instructions
Login by entering your MADI Username and Password and then click “Next”
You will be taken to the ATP “Terms and Conditions” webpage. Click “I Agree” to accept.
You will reach the ATP Participant Portal. This is the End of Step 2.
The ATP Participant Portal is where you will be able to access the COVID-19 survey. Please look under “notifications” or the “Surveys” tab.
