Survey 2017 is Ready!

Thank you for being part of Alberta’s Tomorrow Project. Our follow-up questionnaire is ready online and we need you! Your ongoing involvement is crucial to ensure that ATP’s huge database continues to grow, becoming even more valuable for cancer and chronic disease research.
There Are 3 Steps to Get Started
These steps ensure your private information remains secure and confidential. If you get stuck, don’t worry. We are here to help. You can find some great tips and videos on our website, or contact us directly with any questions.
You’ll need your new account information for Step 2, and each time you log in to work on Survey 2017.
You will then receive a confirmation email from no_replyid@gov.ab.ca. Open the email and click the link to complete your registration.
You will then receive a second email from no_replyid@gov.ab.ca confirming your account creation; please ignore any further instructions in the email at this time.
Please note – We have partnered with the Government of Alberta to use this secure system for registration only. None of your survey information will be accessible or shared with the government.
Step 2: Register to complete Alberta’s Tomorrow Project’s online survey by clicking your personal link below.
Please Note – The links below are unique to each participant in our study, and only the template is shown here. None of the below links will work for access – you will need to refer to your unique email. If you have not received an email yet and would like to get started on Survey 2017 early, please email us and we will send your invite email to the email address we have on file.

This link is for ATP Participant use only.
You may be asked to enter your new MyAlberta Digital ID username and password from Step 1.
Cick Log In.
You will then be redirected to https://myatphub.ca?userPIN=XXXXXX where your unique PIN number will automatically be loaded.
Now click Submit.
Step 3: Read and click I Agree to the Terms and Conditions. Then click the Surveys button on the left and you are ready to start!
Survey 2017 will take you approximately 45 minutes to complete.
If you need a break, come back later to https://myatphub.ca (Step 2) and continue the survey where you left off.
